Dear Patients,

It is with mixed emotions that we write this letter to inform you that we have made the difficult decision to close Blue Hill Pain Care as of June 01, 2024. We will be available to provide you care until May 30, 2024. For those patients who receive opiate (narcotic) medication prescriptions from our practice, another provider will need to assume responsibility for prescribing. Our office can assist with a transfer letter; you can also speak to us about weaning down or off of your medication prior to the practice closing. It is important that you secure a new physician prior to May 30,2024, to ensure continuous care for your condition. For names of pain management physicians in your area, you may contact your primary care physician, health insurance company, the Massachusetts medical society, or a physician referral service at your hospital of choice.

We will be joining The Quincy Spine Center in Quincy, MA in the Fall of 2024. It will be on a very part-time basis to provide ONLY interventional care. Due to the constraints of this new practice, we will NOT be able to prescribe opiate (narcotic) medications . Their phone number is 617-773-6300. Appointments with us can be scheduled for the fall staring August 1, 2024. Please do not contact them before this time.

As of June 01, 2024, Morgan Records Management will become the custodian of your medical records They will be able to provide record copies to you for a fee. Their contact information is as follows:

Phone Number: (833) 888-0061

Website: MorganRecordsManagement.com

The best way to access your medical record is to sign up for our patient portal. Our front desk staff can assist you with this by sending an invitation to your email. You will have access to your records indefinitely through this method. If you are unable to use the patient portal, our office can supply you with one copy of your medical records for a fee. You will need to fill out a medical release form and turn it into our front desk no later than May 1, 2024. Please be patient as it may take longer than usual due to a high volume of requests. We will process record requests in the order in which they are received.

Providing your health care needs has been a privilege. Please know that we have greatly valued our relationship and wish you the best of health.

Sincerely,

Jeffrey Jackel, M.D.                 Cristin McMurray, M.D.
